Documentation updates. Rename mortality prediction in cat_trauma() output to Pmort.

icdpicr 1.0.0

This is the first major release since the initial version of icdpicr was created. It is also the first version that will be on CRAN. The biggest update is a new method for generating mappings from ICD-10 codes to AIS. We have also added the ability to use ICD10-CM or ICD 10 international codes.

Version 1.0.0 changes

• New ICD-10 lookup tables
• Removed default values for icd10 and i10_iss_method in cat_trauma()
• Reordered cat_trauma() arguments
• New vignette describing methods development and validation
