dhelkey/dghrank: Construct Draper-Gittoes-Helkey Institution Rankings

Provides functions for ranking institution quality as measured by to binary performance indicators, accounting for categorical and continious risk adjusters. Rankings computed according using Draper-Gittoes-Helkey (2018) methodology. Functions are provided to run analysis on individual performance indicators and generate composite scores. Main fitting functions (fitBabyMonitor() and scoreComposite()) intended for CPQCC/VON usage, but sub-functions provided for probit and Bayesian linear regression implemented with sparse matrices.
